NOTICE OF DISSOLUTION Bridge of Hope of the Laurel Highlands, Inc., a 501(c)(3) non-profit corporation, with a registered address of 445 Schoolhouse Road, Johnstown, PA 15904, is hereby giving notice that it is dissolving pursuant to 15 Pa.C.S.A. ¤5975. Accordingly, any persons or entities with a claim against Bridge of Hope of the Laurel Highlands are notified as follows: (1) Present any alleged claim in writing to Bridge of Hope of the Laurel Highlands c/o Michael A. Carbonara, Esq, 227 Franklin Street, Suite 310, Johnstown, Pennsylvania 15901; and (2) Any alleged claim submitted must contain sufficient information/documentation to reasonably inform Bridge of Hope of the Laurel Highlands, Inc., of the identity of the claimant and the substance of the claim; and (3) The deadline for filing a claim shall be 60 days after the date of this notice and, therefore, said claim must be received within the 60 days; and (4) Any claims not in writing or not received by the time deadline will be barred; and (5) Bridge of Hope of the Laurel Highlands may make distributions to other claimants or members of the corporation or persons interested as having been such without further notice to any claimant.
