Request for Proposals/ Applications for Fiscal Years 2016-1017 Milestones Area Agency on Aging will be accepting Request for Proposals/Applications for Fiscal Years 2016-2017 from organizations that wish to provide services in one or all of the following counties: Appanoose, Clinton, Davis, Des Moines, Henry, Jefferson, Keokuk, Lee, Louisa, Lucas, Mahaska, Monroe, Muscatine, Scott, Van Buren, Wapello, and Wayne. Request for Proposals will be available for homemaker, transportation, legal assistance, congregate and home delivered meal services, adult day care, outreach, chore, counseling/caregiving/Grandparenting support, health promotion, respite, reassurance, delivery service for meals going into homes, individual meal trays for transport, and food vendor, in one or all of the counties listed above. Also Vendor Agreements for Client Based Purchase of Service are available in specific counties. To be considered, organizations must request, complete, and return the Request for Proposal/Application packet by mail, electronic, or hand delivered to: Milestones Area Agency on Aging, 623 Pennsylvania Ave., Ottumwa, IA 52501or email; pheemsbergen@milestonesaaa.org no later than 4:30 p.m. on January 28, 2015. All requests for Proposals/Applications/Bids are subject to funding availability and we may accept or reject any part of any Proposal, to accept or to reject any or all Proposals without penalty or obligation, to advertise for new Proposals, or to cancel this RFP if it is in our best interest.
