LAVALE SANITARY COMMISSION LAVALE, MARYLAND CONTRACT NO.: 2014-S-01 MANHOLE REHABILITATION Separate and sealed bids will received at a meeting of the LaVale Sanitary Commission at their office at P.O. Box 3325, One Roselawn Avenue, LaVale, Maryland 21504, until 10:00 a.m., Prevailing Time on January 28, 2015 at which time the bids will be publicly opened and read aloud. Under this Contract, the Contractor shall furnish all materials, equipment, supplies, and labor required for manhole rehabilitation to provide for a watertight seal preventing infiltration. The project will consist of the rehabilitation of approximately 700 manholes including manhole linings, grade ring adjustments; frame and cover replacement, step removal, bench repair, and flow channel repair. The project is located in LaVale, Allegany County, Maryland. 1. Contract documents are on file for review at the office of The EADS Group, Inc., 450 Aberdeen Drive, Somerset, Pennsylvania 15501, 814-445-6551and the office of the Owner for viewing by interested bidders. Contract Documents and Plans are to be purchased from The EADS Group, Inc. for the nonrefundable fee of $350.00 for one hard copy set or for the nonrefundable fee of $25.00 for the electronic format set of contract documents and plans, both of which includes 6% Pennsylvania sales tax. To access the electronic format the bidder must have a valid email address and will require an internet connection. All checks shall be made payable to The EADS Group, Inc. Bidders must purchase the Contract Documents and Plans (in either format) from The EADS Group, Inc. to be eligible to bid this project. 2. 3. A non-mandatory pre-bid conference will be held for all Bidders at 10:00 a.m. local prevailing time on Tuesday, January 13, 2015, at the office of the LaVale Sanitary Commission located at P.O. Box 3325, One Roselawn Avenue, LaVale, Maryland 21504. Bid security shall be furnished in accordance with the Instructions to Bidders. All Bidders on this work will be required to comply with the Requirements and Contract Provisions For the Project Financed Through The Maryland Water Quality Revolving Loan Fund and The Maryland Drinking Water Revolving Loan Fund, Department of Environment, State of Maryland, as stipulated in the specifications. All bidders must comply with federal, state, and local requirements. The obligations imposed on the Owner by the financing agency shall apply to and be acceptable to the successful bidder(s). The Contract awarded will be funded in part by the Maryland Water Quality Revolving Loan Fund, Maryland Department of Environment, State of Maryland and by EPA financial assistance dollars. Contractors must demonstrate the Six Good Faith Efforts to award a fair share of work to Disadvantaged Business Enterprise (DBE) firms that are potential sources for any procurement action in the areas of project construction, equipment, services, and supplies by including small businesses, minority, and women owned business enterprises in the bidding and construction process. Documentation of the contractor’s implementation of the Six Good Faith Efforts and commitments made to DBE firms shall be submitted concurrently with the contractor’s bid. Not less than the minimum salaries and wages as set forth in the Contract Documents must be paid on this project in accordance with the provisions of the U.S. Department of Labor, Wage, and Hour Division under the Davis-Bacon and related Acts. Prior to the award of the contract, the successful bidder shall furnish a satisfactory Performance Bond and a Labor and Material Payment Bond to the Owner, in the form contained in the Contract Documents and executed by a satisfactory circular 570 listed surety, each equal to 100% of the total contract amount. The bidder will also furnish evidence of insurance coverage as required by the contract documents. The successful Bidder(s) will be required to fully complete all work within 270 consecutive calendar days from, and including, the date indicated by the Owner in their written Notice to Proceed. The Owner reserves the right to reject any or all of the bids, or to waive any errors, omission, defects, or informality, and to award the contract, as may be deemed in the best interest of the Owner.
